Appointment requests

When you submit the appointment form, your contact and scheduling information is sent to FormSubmit, a third-party form delivery service, which emails your submission to mulberrydentistry@gmail.com. This includes your name, phone number, email, patient status, type of visit, preferred day and preferred time. FormSubmit and the clinic’s email provider process the submission to deliver the request. Read FormSubmit’s privacy notice for its processing and retention practices. Only provide contact and scheduling information; do not include medical details. Contact the clinic if you prefer to request an appointment by phone.

Google Maps

The contact page embeds Google Maps. Loading that map connects your browser to Google, which may receive technical information such as your IP address and apply its own privacy practices. You can also call us for directions.

Website operation

No advertising or analytics scripts have been added to this website. The hosting provider may process technical information needed to deliver and secure the website. Your browser may retain ordinary cache data.
